Eye Exam Checklist - Students place a check in the box after performing each of the eye tests with their patient
(Laminate or cover with a wipeable transparency sheet and provide a washable marker and clipboard)
Eye Chart (letters) – Official copy for the eye doctor office exam
Eye Chart (dotted letters) – Students’ copy (Students trace the letters)

Glasses Labels (Color and Black and White) - Use these signs to label the displays of glasses for sale in your shop
Sheet of Price Tags (Color and Black and White) - Laminate and attach to glasses with string or pipe cleaners. Students can write the prices right on the tags and wipe clean when desired.
Re-usable “Stickers” – The doctor can give these to patients at the end of their appointment! (Color and Black and White)
Add your own props to the mix to make it even more fun (i.e., a pointer stick for the doctor to use with the eye chart, glasses to sell in the shop, plastic mirrors to see how the glasses look, etc.) Additional ideas for extras that you could add to the center are included in the product pages.
