As an Itinerant Teacher of the Deaf, it is my responsibility to teach my DHH (Deaf and Hard of Hearing) Students how to maintain and monitor their FM Systems on a daily basis. In order to collect data for IEP Goals and Objectives, I created this FM System Weekly Time Card so that my student could increase their self-advocacy skills, as well as have a visual of their progress. The Time Card is formatted in a PDF File so that the teacher can print 4 weeks at a time, on one page. The Time Card allows the student to document the dates throughout the week, their name, their grade, whether they transported the FM System between classes, charged the system overnight and stored the FM System at the end of the academic day and/or when it was not in use. The classroom teachers appreciated this, because it held the student accountable for their equipment and usage, as well as gave them data to show parents and/or guardians. Once completed at the end of an academic week, the classroom teacher would give it to the Teacher of the Deaf and I held onto these for data collection purposes.
