Encourage your students to read at home with this ready-to-print Home Reading Log! This form helps track home reading practice for one week. It can be sent home with books as a official homework assignment or just as extra practice. Completed forms are returned to school as evidence of reading completed at home. I used this form weekly for many years in my first grade classroom. It can easily be used with first, second, or third graders by classroom teachers, intervention specialists, Title I teachers, or tutors.
The chart includes a place to list the titles of up to 5 books with the book level and a section is included for flashcard practice. A daily chart next to each book can be checked to indicate the day(s) when each book was read. There is a checklist for parents to evaluate how well their child is able to read the book after practice. A weekly reading goal is included at the top of the page. There is a key for evaluating the completed log with a check+, check, or check-. The teacher and/or parent can make comments at the bottom.
