FREE Homeschool Student and Teacher Identification Card

This is a very simple format for a homeschool student (and teacher) ID card which lists some basic information and leaves a space for a photograph of the student (or teacher). Please make sure your state would except this before using as some states might possibly have certain requirements when it comes to student or teacher ID cards.

One form in the folder contains editable layers (you must have Photoshop or another program which allows you to edit these files or it may not work), and I have included another form - with blank areas -which can be printed and filled in by hand. A picture can be cut to size and added by hand as well. (You may want to laminate these ID cards so that they last the entire school year.)

I have another student (and teacher) ID card form with a cute apple graphic on it and have included that particular form in with a Back to Homeschool activity packet (however, I need to update the date on it and will try to get that done this weekend)! Sorry - that one isn't free, but it WILL be very inexpensive and the updated packet will include lots of other fun activities, workbox labels, and more.
