I use this bookmark to track my students' Sustained Silent Reading. There is space to log daily reading, even if students are not in my classroom. In addition, they must fill in important, Common Core-aligned aspects of the book for our one-on-one discussions.
There are two bookmark options: Fiction and Nonfiction. The back is generic to both. I have aligned these pages so that you can cut the bookmarks without worrying about cutting off text on either side. There are no borders or guide lines so that you can cut this from an 8.5x11 sheet with no paper waste. Cut from the back side (reading log side) to be sure you are not cutting off text.
I print these on color card stock (green and pink for fiction, yellow and orange for nonfiction), and once my students turn them in, we hang them from the ceiling. It's great to see how many books we've read and visitors are always impressed.
