These sheets allow you to track & document various student's behavior, as it unfolds in the classroom from moment to moment! Whether it be for student On Task Behavior, Following Directions, Body in Control (Which could mean many things, such as Using Appropriate Voice Level, Hands to Self, or Body in Chair), & Showing Effort. Also, an Other blank is given, if a teacher wants to fill in & monitor something else.
This resource tool offers a way to document Frequency Data using the various Time Durations of 5, 10, 15, 20, & 25 minute increments. The teacher is to mark either a + Plus (For Appropriate Behavior Demonstrated by the student) or a - Negative mark (For Not Appropriate Behavior Demonstrated by the student) inside the boxes during the desired time duration.
It is suggested to select one or two specific behaviors at a time to track. The sheets are created to document weekly &/or monthly behaviors. The behavior coverage is divided into four 9 Week Quarters & includes the months of the year to be circled.
