This two-page document includes a chart/graphic organizer for recording facts and opinions out of a reading source, as well as a follow-up homework page where students take a fact and formulate an opinion from it. I left the fields blank because these could be used with a variety of topics/subjects. For my purposes, I had students read an article about logging protests for social studies, and record information from the text. For homework, I filled in the fact side with information out of our current unit. It was their responsibility to record an opinion about each. You could also have them take both fields, depending on your students.
