Use these handy forms and worksheets to help get yourself organized for your field trip and gice students a chance to evaluate their trip.

-2 different checklists (I find these especially helpful. When you collect money and permission slips, use these forms to simply "check" that they brought in what they were supposed to. You can put this check list on the front of a folder or an envelope and store all the slips/money/lunch requests together.
-1 Permission Slip that has ALL the important information parents will need to know regarding the trip.
--Field Trip Group Sheet (to help you organize which students are going with which chaperones. After it is all filled out, I always give a copy to each chaperone so they know who is in their group as well as the other groups)
-Field Trip Information Sheet (A very thorough sheet to help you stay organized! You can use this to keep all the necessary information together!
-Field Trip Name Tags (4 per sheet) Just print and fill out! It's as easy as that. I punched a hole at the top of each for my class and used yarn to make it into a necklace.
-Field Trip Expectations Sheet (Poster) It is important that before you go out on your field trip, you review what behaviors are expected from your students.
-Field Trip Expectations Sheet (same sheet as above, but this one is sent home and requires students and parents to review. The sheet needs to come back to school signed by both students and parents.
-KWL chart to use before you go on your field trip and complete after the trip.
