This Fifth Grade Positive Behavior Matrix is utilized each day in my class as a way to track behaviors in three different categories: respect, responsibility, and safety. If an infraction occurs, the student figures out which category to fill in and completes a short explanation. At the end of the week, the student must evaluate how he/she did using the self-evaluation at the bottom of the page. The parents must sign this, and the students bring it back on Monday. This behavior sheet can assist with parent communication and in identifying trends. If a student goes a whole week with a blank sheet, the student may decorate the sheet with stamps, drawings, or something else to signify a perfect week.
