If your district requires you to seek parental permission to show films in class, this template can make your life easier!
This single page template allows you to quickly create a film permission slip for one or multiple films that you might use during the school year. It includes a place for the student name at the top (created for easy filing), an introductory statement, a table containing fillable MLA-style works cited entries for each of the films, and a place for a parent or guardian signature.
The form itself has currently has room for five films. By copying and pasting or by deleting, you can customize it for your own needs.
Since I have a good idea of the major films that I will show during the year, I like to complete this form and send it home with the beginning-of-the-year paperwork. I add more films to it than what I think I will show, just so that I don't have to repeat the process later in the year if my plans change. Sending it home in September also allows me to contact parents immediately if they have indicate concerns about specific films in order to determine what the exact nature of the concern, whether if it can be addressed, or if an alternative assignment should be provided.
