First days are hectic enough without wondering how little Billy Bob is getting home or whether or not Sally Sue can eat the el-cheapo snack you brought in for the class. That's where the First Day Student Information Sheet comes in handy.

Most schools have either a Preview or Meet & Greet Night where students can see the classroom, meet the teacher, and drop off non-existent supplies. After meeting the Mansons and finding out way too much about their family background, just hand over one of these sheets and ask them to complete it and send it in with little Xavier on the first day. It has everything you need to make sure your first days go smoothly.

I keep them in my teacher binder all year for quick access to pertinent information and to prove that little Johnny was supposed to ride the bus and not stay for the Spelunking Club like he said. It's a Word file that you can modify to meet the needs of your classroom.
