This two page handout (or one page double-sided) asks students to record information teachers need to get to know their students a little better (e.g. preferred name, timetable, interests, likes/dislikes, and goals). It ends with instructions for a goal-setting paragraph, which, as an English teacher, I use as a diagnostic to assess writing levels of my new students. This allows me to create some mini grammar/writing workshops based on the needs of my class.
