The first few moments of the (first) school day can be pretty chaotic! With all the excitement squeezed into the arrival bell-start bell time period, things can get perhaps a little more exciting than you'd planned rather quickly.
I always like to have this survey set out at every student desk before they arrive the first morning, along with a sharpened pencil. This gives the students something to do as everyone filters in, as I am greeting students/parents, helping to unload supplies, and answering the ever-flowing stream of questions.
The survey includes three pages centered all around the student! It includes questions about hobbies, favorites, outside of school activities, school subjects, things they have and haven't liked about previous school years..etc. The last page includes a space for them to create an illustration about the first day of school.
Not only does having this ready save me some serious sanity, but it is also a great first writing sample to include in my students' files. Lastly, I've found that having something to work on helps to ease the "first day jitters" as they come in :)
