This survey is a great first day of school activity for middle/high school students. It gives me a LOT of information that I can use immediately about my students and their interests, learning styles, and attitudes. I teach English, but you could easily change the subject to Biology, Algebra, etc. This gives students something to do while I take attendance and try to learn names on the first day. I use the information to incorporate their favorite singers, TV shows etc. into my worksheets, to see what learning styles my students have, to learn if anyone has any test anxiety, what skills they have when it comes to reading textbooks, and what their expectations are for their grades in my class.
