Get to know something about your students without having to ask them personally. This is an excellent way to immediately build trust with your students. Excellent first week exercise. Questions are designed to elicit information from the student that may assist you in teaching them and ultimately you class. Tell the students that you will not share any personal information or potentially embarrassing information contained in the questionnaire without their permission. You'll be surprised at what they may tell you. Give the students 15 to 20 minutes to complete the questionnaire.
Make an honest effort to read all of them and write appropriate encouraging comments on the questionnaire and eventually give them back to the student. You can also assess your students' writing, spelling, and grammar abilities. You need not give them all back at one time but over the course of several weeks.
Additionally, you may complete the questionnaire yourself or create a simple power point presentation telling them something about yourself.
While you probably cannot compel students to complete the questionnaire, as an incentive offer extra credit points.
