Resource Directions:

First Grade Quick Assessment Form / Editable Version / Student Sheet: This assessment form is a very quick assessment to be done the first couple weeks of school. It provides a quick snapshot of where your students are with letter recognition, sounds, sounding out word and reading connected text. This data give you information on where to begin your instruction and an idea of where students fit in guided reading groups.

First Grade Rhyming Assessment Form / Editable Version / Student Sheet: This assessment form is a quick assessment of student’s ability to rhyme with all 5 vowel sounds included. It provides you on whether your students can read rhyming words and whether they may have difficulty with certain vowel sounds.

First Grade Reading Screening Form / Editable Version / Student Sheet: This assessment form is based on state testing for student fluency on letter recognition, phonemic segmentation and nonsense words. Students are given one minute to do each assessment. Student reads from their form while teacher marks student’s accuracy and number correct in one minute time.

First Grade Assessment Form / Editable Version / Student Sheet: This assessment form is used throughout the school year to assess student’s letter recognition, sounds, sounding out, reading, sight words (color, number, sight words & red words). Note: Red words are irregular spelled words.

Students can read off sheet provided during assessment period. My students also had their own sight word booklet they kept in their reading folder that had each of these assessments and could be practiced throughout the year. This resource can also be found in my store.

This assessment form is given to parents each marking period so they can see their child’s progress. A classroom data form (found in this resource) is filled out each marking period so data can be assessed to address instruction and student needs. A student comparison form (found in this resource) is also filled out each marking period so student’s progress throughout the year is charted.

First Grade Reading Street Unit Words Form / Editable Version / Student Sheet: This assessment form assesses student’s Reading Street high frequency sight words for each unit. Student’s can be assessed after each unit or throughout the school year.

Student Reading Conference Form / Editable Version : This form is used during student conference sessions throughout the school year. It goes along with the Daily 5 and Café reading strategies. Student’s goals, strengths, conference date, book title& level, teacher observation, strategies used or taught and what to do next are all recorded during conference session.

Student Writing Conference Form / Editable Version: This form is used during student conference sessions throughout the school year. Student’s goals, strengths, conference date, book title& level, teacher observations and instruction and the next step to meet goal or change goal.

Literacy Café Menu with Standards: This form goes along with the Daily 5 and Café Reading programs. It lists reading strategies and the core standards that would be met with the achievement of strategy. A form is made for each student and kept throughout the school year. During a student’s reading conference highlight strategies that have been achieved and discuss which strategy/goal student would like to work on.

Reading Conference Assessment Rubric / Editable Version: This rubric can be completed after a reading conference with student. Students are assessed on a point systems on mini-lesson, reading & conferring; journaling and take-home reading performance. Student's preformance level may be highlighted or circled.

Reading Journal Assessment Rubric / Editable Version: This rubric can be used to assess student’s reading journal. It is based on a point system and assess students comprehension; application of mini-lesson; completeness, personal response, and writing convention. Student's preformance level may be highlighted or circled.

First Grade Writing Rubric / Editable Version: This rubric can be completed after writing conference with student or to assess a writing piece of a student. Students are assessed on ideas, organization and conventions. Students are then given level of writing which include: Excellent, Competent; Developing or Emerging Writing.

First Grade Trimester Report Form / Editable Version: This report is given to parents with their child’s Trimester Report Card. It is a record of their child’s achievement academically and socially. It is a form that shows parents their child’s grade, skill level, strengths and areas that need improvement in each academic subject and behaviorally. Student's preformance level may be highlighted or circled.

First Grade Marking Period Report Form / Editable Version: This report is given to parents with their child’s Marking Period Report Card. It is a record of their child’s achievement academically and socially. It is a form that shows parents their child’s grade, skill level, strengths and areas that need improvement in each academic subject and behaviorally. Classroom Assessment Data Form / Editable Version: This form is filled out after the beginning of the year student assessments and for each marking period. It gives an overview of how students are doing overall with their reading level and sight word knowledge. It can guide you on your instruction, on individual student needs and whether guided reading groups need to be changed or whether students may need other services available in your school district.

Student Trimester Assessment Comparison Form / Editable Version: This report is given to parents with their child’s Trimester Report Card and assessment report. It allows you and parents to see how individual students are achieving throughout the school year. It helps you work out a plan to help students who may be struggling or to give those student who are high achievers more advance instruction.

Student Marking Period Assessment Comparison Form / Editable Version: This report is given to parents with their child’s Marking Period Report Card and assessment report. It allows you and parents to see how individual students are achieving throughout the school year. It helps you work out a plan to help students who may be struggling or to give those student who are high achievers more advance instruction.
