I use this fluency intervention lesson plan template to use daily with my fluency intervention groups. These groups are formed of the students struggling most with reading fluency.
Each day, we focus on a different part of reading fluency or a specific drill.
Mondays and Wednesdays, we do high frequency word and phrase drills. Tuesdays and Thursdays are for speed and accuracy, which can be more focused on individual areas of struggle. On Friday, we focus on prosody, intonation, and pacing. This is where I talk about pausing at punctuation, etc.
This template serves as documentation that a certain intervention was introduced to a student and gives you space to record notes on student progress.
