I use this form for my student-directed fluency center during my reading rotations. I use this in conjunction with a fluency kit that I purchased. Students listen to a story on my iPad, read with the recording, practice reading by themselves, and then read out loud and get timed by a partner. They also answer a few comprehension questions with a partner. We do not record accuracy, but rather focus on expression, fluency, and rate of reading. Students record their progress on this form and what activity they will do the next time they visit this center based on if they reached their goal or my benchmark for each given level. I keep all forms in a binder so that I can check daily progress.
