I use this form to track the reading conferences I hold with my students. For each conference, there is a space to write the date, the title (I usually put the level right next to the title as well), and spaces to track accuracy, fluency, and comprehension. In addition, there is a section on the right side to note the goals you discussed in the conference with the student. Note: the "i/o" in the accuracy section stands for "instead of". If I child says has instead of had it would be recorded as has i/o had. The S-C stands for self-correction. I simply keep a tally here; each time a student self corrects, I make a tally mark.
