Form to Track On Call Coverages (Missed Planning or Preparation Periods)

Do you ever lose planning time to cover for another absent teacher in your school? Is that preparation time owed back to you under your union's collective agreement or school contract? If so, it's in your best interest to track those lost prep periods as well as when they are paid back.

I made this form for my own personal use when the sticky note system I had was overflowing in all directions, and now I'm sharing it with other teachers who might have the same arrangements. Print off a copy or two & place it in the front of your planning binder, or even tack it to a bulletin board near your desk.
