I teach math, and I use this form to track student learning with each lesson. (I have used it in other content areas as well.) I fill in the chapter and lesson number at the top, as well as the questions from the chapter test that correlate to the lesson. I also include the Common Core “I can” statement.
I type each student’s name in the box, and as I meet with students, or check their practice sheets, I code their level of progress along the stairs. When I give the chapter test, I do the item analysis right on this form. In the upper right corner of each box I record the number correct for each skill. For example, if there were 5 test questions from a particular lesson and the student got 4 correct, I note 4/5 in the corner. Then I highlight the star with green because 80% meets my district’s criteria for mastery. At a glance I can see which students have mastered the skill and plan for continued practice for those who didn’t.
