Formative assessments are used in the classroom to get a pulse on the learning of your students. Teachers take these quick assessments and then use them to guide and enhance their instruction. After teaching a concept and believing that your students have grasped the knowledge your standards require - distribute this formative assessment to see if students gained the the understanding you hoped they would.

I created this formative assessment on a half sheet - and then duplicated it as the same in the space below. I liked to make a short stack of copies of these - then cut them in half - and then accessible to me if/when the need to assess my students would present itself.

This formative assessment is easy to use, self explanatory, and then a quick read for the teacher. Students will make a list of the five most important ideas of what they learned during your lesson. The teacher collects them - maybe does a sort - or learns if there are any gaps in his/her students' learning. I never took grades on formative assessments.
