This is a quick-and-easy assessment to guage a student's understanding of a reading and assess their basic reading skills as determined by the ELA Common Core Reading Literature Standards (#1-6). After students read a grade-appropriate short story, chapter, or even a full text, they answer each numbered question as thoroughly as possible. Because this assignment is standards-referenced, the quality of the response is a good indicator of the student's proficiency in the respective standard. Follow Up questions are there for students to attempt to demonstrate advanced proficiency.
The only thing you have to do is select a short story/chapter and vocabulary word(s) that students can interpret using context (for R4); input selected word(s) in the designated spot.
This assessment tool also comes with some look-fors for each question which can be used as a checklist to help score your students' responses, ensure they supply a complete answer, and provide feedback with certain aspects they might be missing. I use this Look-Fors document in my instruction to guide my mini-lessons & re-teaching; I also provide this document to students as a guide to help them understand how to meet the standards.
