This foldable notes page can be used to teach or review various forms of linear equations. Forms included are slope-intercept, point-slope, horizontal line, and vertical line. There are 3 versions of the foldable with varying amounts of information filled in. Each tab has space to write and explain the form, and there is an example problem using each form. A key is also included.
My students keep notes in a 3-ring binder, so I designed this foldable to be hole-punched and put in their binder. It would also work well as part of an interactive notebook or simply as a handout.
