Product Description
I have my students fill this information sheet out on the first day. I keep them in a binder so that I can easily call home or track them down at school. It is a great back up when the technology fails. When I make personal contacts with parents, I jot down the time and date and number on the back of the page. This comes in handy during parent/teacher conferences.

The planner sheet is a simple calendar for students to jot down their homework assignments for the week, or to plan out a project.
