Here is the format I use to teach vocabulary. This helps students use context clues to guess definitions of words, repair their comprehension through reading the dictionary definition, find synonyms and antonyms to help them work with the definition further, then finally practice using the word in their own sentence. I usually use this in combination with novels I am teaching, but you can use this in a variety of ways. I simply fill in the "word" and "how the word is used in the text" boxes—the rest is up to the students! I usually give them a definition sheet after they make all of their context clue predictions, but you may prefer to have your students use dictionaries or electronic devices to find the definitions on their own. Also, this is designed for ten words, but simply copy and paste to add room for more words. I hope you enjoy this free resources—I love how it doubles as the students' study guide for their vocab quiz!
