If you like this product, please rate it so I know to post things like it! This is my take on the ILP (Individual Learning Plan). It is the most detailed, most inclusive, and most comprehensive ILP I've ever seen. I use this ILP to document assessment data in one place and make decisions about how to meet my students' needs. I also show this document to parents during conferences to show them students' strengths and weaknesses. I have broken down subjects (Reading, Writing, Math and Life Skills) into sub-categories, but these can be changed to meet your needs based on district curricula, state tests, and grade level. The "Current Skill" column cells can be color-coded; I use rainbow colors to color code the cells (Purple for advanced, blue for above average, green for target, yellow, orange and red for different levels below target). A quicker way to code these cells would be to mark them with letters based on your grading system. The
"Strategies" column is where I make notes of programs, small groups, and strategies I can use to intervene in or enrich the student's learning. This document is in Word format so you can adapt it to meet your needs.
