Free The Hunger Games Permission Slips for Reading & Movie

Description
Though I wish this wasn't necessary, due to several requests from teachers, I'm providing a free download of a reading permission slip for The Hunger Games novel by Suzanne Collins. There are two different forms, and you can also customize them to suit your needs. Provided in both Microsoft Word and Adobe PDF.
I also included tips and rationale for teaching the novel, in case it is challenged at your school.
A third letter is a permission slip to go on a field trip to see the movie "The Hunger Games." It can also be edited so it can be used for viewing the DVD when that comes out in August.
