There are 2 versions here, one junior and one senior. Copy back-to-back and fold on the fold lines to make a booklet.
With the key words and resources on the exterior, students are reminded that these are what they record first.
For seniors, there is space for 3 resources to encourage triangulation (confirming facts in more than 1 source). Then the interior is where they write the paragraph, in their own words, from their key words and phrases.
Students collect a new sheet for each paragraph of their explanation / information report. This way they can continue on with their work while waiting to conference with the teacher on completed paragraphs.
