The G-PAR Form was developed in response to my students needing a way to set personal goals for their academics or behavior. G-PAR stands for Goal, Plan, Action, and Result and is a component of William Glasser's Choice Theory. This form also includes a first step of "reflect" which allows students to first think about what behavior they want to change before creating a goal. It provides teachers with an option to set unofficial behavior plans for student self-improvement.
The G-PAR form comes in plain black and white and also in a few colored frame options. The colored options also print well in gray scale which adds to the visual appeal of the document.
