Simple form for students to complete. This is an indicator of levels of stress and anxiety they may be feeling at the moment. Ask them to tick the boxes and total up the scores. The columns have scores attached so it is easy to see how to score your own chart. Good to use in a group, especially when you ask them not to fill in their name and just add the whole group scores up to get a baseline for you class. Use this with any of the mindfulness resources and also the 'The Chat' resource which is all about attendance and missing school days.
