After using this checklist in my classroom, I saw that this was an excellent way for students to TAKE OWNERSHIP of their own learning and also a way for me to see which students need extra support.
HOW THE CHECKLIST WORKS
After returning a class assessment/independent work, students will check the standards-based questions from the assessment. If they answered the question correctly, they can fill-in the "GOT IT" box. If they missed the question, they will write a PERSONAL GOAL in order to show that they will work on understanding the standard. Examples for creating goals can be: retake a test, before or after school tutoring, extra homework assignment, etc. Once they have completed the goal they can check the second "GOT IT" box.
