A wonderful ice breaker that gets students moving and interacting, and as a bonus, gives you important details about their learning interests and preferences.
Day 1: Have your students complete the questionnaire. Once you've collected them, go through them and find something unique about each student. Type the information into the BINGO card Excel document (see related BINGO Card Creator Excel document) then print and have the cards ready for Day 2.
Day 2: Pass out the BINGO cards and have students go around the room getting to know their classmates. When they find someone who "owns" the fact in one of the BINGO boxes, have them write that student's name in the box. NOTE: I recommend that this is NOT used as a competition! Instead, make it so that every student who completes a BINGO card is a winner. Establish right away that doing the work pays off for everybody! Each student who can accurately identify the owner of each box gets a treat!
