I teach in a junior high setting. So I have each student complete this info sheet that first week of school. I hole punch them, group them by period class, and keep them in a binder. I add data throughout the year like reading levels. I keep the binder where my intervention specialist can grab it, use it, add to it. I pull it out at parent conferences or meetings to show I have taken steps to get to know the student and have a plan for that knowledge.
