The goal of the questionnaire is to help teachers get a better idea of their students’ personal and academic interests as well as their home life. The questionnaire is intended to be informative and also fun. People love to talk about their interests when given the chance, and this questionnaire definitely makes it easy to open up discussions, share ideas, and make connections.
I came up with the questionnaire last summer and used it with my 7th grade advisory at the start of the 2016-2017 school year. The students really enjoyed it, and I ended up using the questionnaire later for IEP interviews. my students and I always have fun with it, and I always viewed it as a fun icebreaker. The questionnaire helped me build better relationships with my students.
The questionnaire is 4 pages long, with 50 questions on 3 pages, plus an additional 20 blank spots on the back page for students to write down fun things that make them happy. There is a PDF and a Word version, so you can edit the questionnaire. I also include an implementation guide with how I've administered the questionnaire in 1 on 1 interviews as well as small and large group.
