This is a simple worksheet I use for my ESL upper-secondary (high-school) students to learn more about them. It's aimed at teenagers/intermediate ESL students.
It is primarily a fill in the blanks from prompts that ask about such things as the following:
My best skill
Goals... (an so forth)
The second part of the worksheet asks the students to look at their friends answers and to write about their friend. This is useful practice for present simple grammar (e.g. She likes...).
It's a good way to start the year with a new class by getting to know what motivates your students. Then you can plan future lessons around their interests.
The worksheet can be edited.
There is no answer key as the students will be writing different things according to their own thoughts.
I use this worksheet at the start of every year with grade 11 ESL students.
