This is a terrific handout for me that I always give out on the first day of school It is a one page questionnaire that tells me a lot about my new students. I can read through them and get a feel of how the culture of the class will be. After a couple weeks in the classroom when I have had the chance to learn my new students names, I look at these again. I try to match up their interests with them when I talk to them. It really helps early on to build relationships that otherwise might take many months. The fact that they are memorable enough for me to talk to them about their interests usually helps a lot in building community and classroom management.
I have used this in class two different ways:
1) I have had students take it home after the first day as their first "homework" assignment to practice procedures on homework.
2) I have had them fill it out in class and then talk with a partner and share what they wrote. Then I have the partner introduce their new classmate. It is a great way to have introductions without the shyness of introducing yourself to a room.
I also have one filled out and model it on the Elmo so my kids can see a little bit about me and about my expectation on how they should fill it out.
