Exit slips for each lesson (1-10) in Go Math Chapter 7, grade 4. There is a lower level with scaffolding, models, or tips and a grade level slip for each lesson.

I was struggling with kids doing well on their homework but then bombing the chapter test. I couldn't figure out what was happening. When they did well on their homework I assumed they understood, so I wasn't pulling those students for MTSS. Using exit tickets, however, has been an amazing way for me to see right away who is struggling with the lesson material. It has been a game changer. I feel like I'm able to target the right students with the right interventions in a timely manner. The homework just wasn't giving me good enough data. I will purchase more exit tickets for each chapter - that's how transformative they have been for my instruction. I can't imagine going to back to just relying on homework to see if kids are "getting it." This particular exit ticket item lines up perfectly with each Go Math chapter. Fantastic resource.
