We started using "Go Math" last year and added the online component this year. While I love all the information it gives me on how each student is doing, it can be a little overwhelming too. So, I created a few forms to help me see my class "at a glance" to know who needs what. One "all in one form" is for one lesson's data. It is made to track how each student performed on the lesson online, on the lesson check, to track whether or not they were pulled for a reteach group, and to mark if they still needed the personal math trainer (online), and add their score. This page is intended to be a space for you to see all the data for one Go Math Lesson (online and/or paper-we use both) at a glance. It also will show you who needs follow up help, whether they got it yet or not, and has the space for you to say whether they still need more practice so you can remember to assign that later. In addition to the "all in one form" that tracks a lesson at a time, I have also included a form for tracking an entire chapter (up to 14 lessons) all on one sheet for up to 25 students. I also have a form just to see your class' performance on the pretest (show what you know), mid chapter check, and the chapter test. I like this one for seeing my students' growth over a longer period. Lastly, there is a page for you to just jot student names to help you form groups and plan your next steps for each group.
This version is a PDF created to keep the cute fonts by KG fonts and table formatting by me. If you would like the Word document as well so you can make changes on your own or type in student names, just send me a message after purchase with your email and I will send you that one as well.
