Having the appropriate documentation and a sufficient paper trail is very important throughout the AIS/RtI process. Knowing, communicating and following a student’s goal plan is very important to their success.
This simple form will establish a formal document to follow when implementing student goals. Ideally, as an instructional team, decisions regarding student goals, interventions and progress monitoring will be decided. This form could be added to student files, shared with interventionists/classroom teachers, or sent home to families.
In following with RtI guidelines, I progress monitor my Tier 3 students for 6-8 weeks on their specific goals. Following the 6-8 weeks of data collection, I graph the data and attach it to this goal sheet. I can note whether the goal was met, or should be adjusted. Then this documentation helps to inform decisions made at our next team meeting.
This form is editable in PDF. You can use the fillable categories while at an instructional team meeting. Or if you are a paper and pencil person, you can fill the general information in ahead of time, and print off multiple copies to fill out later.
