This is a great way to help students set goals hat focus on their academics and behavior/work habits. There is a space for students to set their goal, write the steps it will take for them to reach their goal and list things that could possible keep them from reaching their goal. It's amazing to see how honest students can be when they put this in writing. The last part really helps them realize some of the things that can keep them from being successful.
The back gives them a space to draw themselves reaching their goal. It's important for them to be able to visualize what it will look like when they succeed! There are also spaces they or you can use to check in on their progress. Sometimes I would have the whole class look at their sheet and write about their progress. Other times I might meet with a student who is struggling and fill it out with them to help get them refocused on their goals.
