This is a set of documents intended to provide a simple, quick way for students to set personal, behavioral, or academic goals and track their academic progress. Great for use with an advisory class or as an end-of-the-week reflection activity for any class. It includes the following:
- Quarterly Goal Setting Form: Two pages, one for each goal, with prompts for students to set a goal, explain why they chose the goal, and indicate how they plan to reach the goal; also an end-of-quarter reflection prompt.
- Weekly Reflection: An open-ended form with prompts for students to reflect on the best and worst moments, efforts, and grades of the week. Can be used in conjunction with or separately from the Quarterly Goal Setting Form.
- Grade Tracking Graph: A blank line graph for students to record grades for any class on a weekly basis. Designed for a nine-week quarter with a 0-100 letter grade system.
- Final Scores and Grades: One page with tables for students to record MAP scores, Lexile scores, and their final quarter grades for each class.
