Every year I have a student who needs to work toward an individual goal of some kind. Most often, it is working toward better grades and/or putting in their best effort to assignments turned into me.
I created this simple tracker as a way to work WITH students and families at home to help my student make progress! There are two goal sheets on each page, so you could cut them in half for your students, or give each student a whole page to work toward two separate goals.
Help your student(s) set their specific goal. You can set a planned end date, or just fill in the final date when the goal is met. For each step toward the goal, a student gets to fill in the box with an "X", a smiley face, a sticker, initials or just a check mark.
Sometimes, I will give a reward of some kind in my class, but more often than not, parents want to reward their children at home for their progress! I always tell parents to interview their child to find something they'd really like as a reward. And it should always be something the parent is okay with rewarding. For example, if they don't like to reward with food, then a trip for FroYo wouldn't be a great reward. :)
Please let me know if I can do anything to make this sheet easier to use.
