This chart is a helpful way to track positive behaviors. Here is how it works ,first the student selects a goal from the back ( this is a 2 page document intended to be copied front to back). Then the chart is kept on their desk. When ever they are caught performing their goal they can give themselves a check. This encourages them to continue the behavior as well as gives them an awareness of the replacement behavior you are shaping and teaching them.
After 5, 10, 15 or the entire sheet is filled they can get a reward. You manage the rate of the reward and what reward. I usually use our class token system, or a very small treat.
Never remove a check, this is not a response cost system.
Many students are highly motivated with this chart. It provides a means for verbal and visual positive reinforcement and can be easily differentiated. I hope you find it helpful. Please let me know if there is a goal not listed you would like. I also left the bottom portion blank for you to write another individual goal.
