These 2 pages are quite useful for that student that needs hand holding to complete tasks. I created this for my students keep them accountable for completing a few assignments by breaking them down into smaller pieces. This can easily be modified for any project. There is a spot for the students to sign off that he/she completed each task, and a spot for the teacher to sign off (after the fact) that the tasks were indeed complete. A parent then signs the form so they can see the level of differentiation that you had done/needed to do with the child.
There is also a blank template in which the student writes in their own goals each day or writes down several goals/tasks and decides (with the teacher's guidance or assistance) when each task is needed to be completed by.
