As I have traveled around Western Canada, one question that kept occurring was “How do I assess where my kids are and how to I assess what they know?” Frustration occurred because students were lacking background information, but teachers did not know what information was lacking. I created this assessment to help teachers assess what students know prior to teaching outcomes, and what they students know after the outcomes have been taught.
The Transition Assessment was create to help teachers do the following:
• Assess where the students are at the end of each unit or grade
• Assess where the students are at the beginning of the year
• A communication tool between transitioning teachers
This Transition Assessment is divided into 3 sections: Numbers, Shape & Space and Patterns & Relations. Only the skills related directly to the following year's curriculum are assessed.
