This 50 question math pre-assessment is based on the grade 4 and 5 curriculum expectations from the Ontario Curriculum. Think of it like a KWL for math. You will find out what the students know and need to learn.
You can use this pre-assessment any way which you see fit! Use your professional judgement to make this product fit the needs of your students in your classroom.
The first page of the pre-assessment is a survey designed to gauge your students feelings about math. You can give students this page at any point.
The pre-assessment includes 50 questions, 10 from each strand in the Ontario Curriculum Mathematics for Grades 4 and 5. There are 10 questions per strand (Questions 1-10 are from Patterning and Algebra, 11-20 from Number Sense and Numeration, 21-30 from Geometry and Spatial Sense, 31-40 from Measurement and 41-50 from Data Management and Probability). You can administer the assessment all at one time, or break it down into a strand a day, or use it before you teach a unit. It’s up to you!
I must stress that these questions do NOT cover all of the specific expectations for each strand, but rather give a SNAPSHOT of what students know and can do. As well, students are not expected to know all of the answers as the questions address some skills and knowledge that may be new to them. The information you gather will assist you with planning for your whole group instruction as well as for small groups and for guided math.
The student summary sheet included at the back of the package is intended to allow you to create a holistic picture of where the student is at the time of the assessment. It will give you the opportunity to capture strengths, areas for improvement and identify next steps.
