I used this with science, but it could be used in any subject. Once per quarter, the student could earn 5 points extra credit for filling out the top and bottom of the form. They would select items that they thought could help them do better. This had a dual benefit as it gave the students a sense of empowerment over improving their grades and ideas on how to do so, but also was VERY helpful at conferences when parents came in with concerns. I could pull out the bottom half of the sheet and show the parents what the student had decided they needed to do to improve their grade.
