This form helps the teacher receive valuable feedback from students about what works and what doesn't work in the classroom. The form has two sections that are a page long each. The first section has three questions students answer about the teacher. The second has four questions students answer about themselves. A shorter half page version of the feedback form is also included. An informational sheet for teachers has hints about how to use the form effectively in the classroom.
