This editable document can be attached to the front of a student's Signed Papers Folder for parent's to sign. A parent's signature would verifiy that the parent(s) saw the student's grade(s). This form also includes spaces needed to record weekly grades that are considered failing or lower than a student's normal grade range on a weekly, more consistent basis which is one of its main benefits. Another benefit is that it provides documentation that the parents were aware of their child's grades and it also keeps the students accountable by having the lower grade(s) recorded on the form so they wouldn't feel the need to hide lower grades because they know their is consistent communication between the teacher and parent. A parent's signature on this weekly form would be ideal documentation during a parent-teacher conference as well especially as some schools are approaching the end of their school year and retention lists are in progress. Lastly, it can be used for the whole class or on a student by student basis if every student in your room doesn't need this. It also helps by providing a log of a child's lowest subject area as the grades will fall column by column or for TST intervention for school counselors.
